The equipment utilized in Film & Television (FTVNM) is of such a specialised nature, that maintenance alone is a costly and time-consuming venture. The replacement of damaged equipment may not always be immediately possible due to wider school budget constraints and other financial considerations. Please then, make it a priority on your behalf to ensure due care is taken when borrowing any FTV equipment. Do not hesitate to report any problems with the equipment as soon as possible, so the item can be assessed and put back into circulation as soon as possible. By signing this indemnity form you are officially and legally agreeing to the terms stated.
I _________________________, hereby agree to be responsible for any items of FILM & TELEVISION equipment that I may borrow during my time of studying this subject at Cairns State High School. I have read and acknowledged the relevant information pertaining to the use and care of the equipment, and will not allow any persons outside of myself to either use or handle the equipment without the proper written authority. I will ensure the correct storage of the equipment at all times and keep all accessories and peripheral devices enclosed within the appropriate cases/storage containers. It is my sole responsibility to return all equipment by the said due date, with the battery fully charged (where applicable).
I understand I cannot borrow equipment without first booking it out through the FTV BOOKING REGISTER.
